GRINDING & JAW

Bite plates and night guards

For grinding, clenching and jaw pain: what a bite plate really does, and when you need one.

Bite plates and night guards, VitalDent Secunda

Most people who grind their teeth don't know they do it. It happens in their sleep, and the first witness is either a partner who hears it or a dentist who sees the wear. If you wake with headaches or a stiff jaw, or your teeth look shorter and flatter than they used to, this page is probably for you.

A bite plate (also called a night guard) is a custom-made cover, usually worn over the top teeth at night, that takes the grinding instead of your teeth taking it. It's the standard treatment, it works, and it's honest work rather than magic, which is worth understanding before you buy one anywhere.

How you'd know you grind

The common signs, any of which is worth mentioning at a check-up:

  • morning headaches, often at the temples
  • a jaw that feels stiff or tired when you wake
  • teeth that look flattened, chipped at the edges, or shorter than before
  • sensitivity or aching in many teeth at once, rather than one
  • a partner who hears the grinding at night
  • scalloped edges on your tongue or a ridge line inside your cheeks, from pressing

Not everyone who grinds has all of these, and some of them have other causes, which is why the diagnosis is a look at your actual teeth, not a checklist. The wear pattern on your enamel tells us more than any questionnaire.

What a bite plate actually does

The truth first: there is no cure for grinding. It's linked to stress, sleep and factors nobody fully controls, and no appliance switches it off. What a bite plate does is manage it well: the plate takes the wear instead of your enamel, spreads the load so the force stops concentrating on individual teeth, and eases the strain on jaw muscles and joints. Many people find headaches and jaw stiffness improve; your teeth stop getting shorter either way.

That's the honest deal: the plate is a shield, not a cure. Enamel doesn't grow back, which is what makes the shield worth having before the damage, not after. Grinding wear that has already gone deep is repaired with fillings or crowns. Far better to protect what's left first.

Daytime clenching deserves its own mention. Long shifts, deadlines and concentration push the jaw shut for hours. Desk workers and shift workers know the pattern well: you catch yourself with your teeth pressed together and your jaw braced. A night guard doesn't fix daytime habits, but noticing is most of the battle. The rule of thumb we teach: lips together, teeth apart. Your teeth should only touch when you're chewing.

Jaw pain, clicking and TMJ

The jaw joint (the TMJ, just in front of your ear) takes much of the load from grinding and clenching. Trouble there shows up as pain or tightness in front of the ear, clicking or popping when you open, a jaw that locks or won't open fully, or pain that spreads to the temple and neck.

An occasional click without pain is common and usually harmless. Come in when there's pain, limited opening, locking, or a click that has changed. Those we assess properly rather than guess at. Treatment starts conservatively: a bite plate to unload the joint, habit changes, soft food during flare-ups. Most jaw-joint trouble responds to exactly that, and we'll say plainly if yours looks like it needs more.

Custom-made vs pharmacy boil-and-bite

A pharmacy boil-and-bite guard is better than nothing, and for a short trial (you're not sure you grind, you want to test whether a guard helps), it's a fair, cheap experiment. The honest comparison beyond that:

A boil-and-bite is bulky, fits approximately, and tends to come loose at night or end up on the pillow. Some people also clench harder against the soft rubbery material. It wears through quickly if you grind heavily.

A custom bite plate is made from an impression of your own teeth by a dental laboratory. It's slimmer, it stays put, the bite surface is adjusted by the dentist so your jaw closes evenly on it, and it lasts years rather than months. It costs more upfront; per year of use the difference shrinks.

The process is simple: an impression at the practice, the laboratory makes the plate, and you come back for a fitting where we adjust it until it sits right. Fitting is usually about one to two weeks after the impression.

What it costs

A custom bite plate is one of dentistry's more affordable pieces of insurance, a fraction of the cost of rebuilding worn teeth later. You'll get the exact price before we take the impression. Some medical aids cover bite plates and some don't; we'll help you check yours first.

Frequently asked questions

How do I know it's grinding and not something else?

You often can't, from the inside. Sinus pressure, a cracked tooth and jaw-joint trouble can all mimic it. The wear on your teeth is the giveaway, and that takes an examination. Book a check-up and say you suspect grinding.

Will a bite plate stop the grinding itself?

No, and be wary of anything that claims otherwise. It protects your teeth and eases the load on your jaw while the grinding happens. For many people that's enough to end the morning headaches.

Is it uncomfortable to sleep with?

The first few nights feel strange. Most people stop noticing it within a week or two. A custom plate is slim precisely so that it disappears from your attention; the bulk is what makes pharmacy guards hard to persist with.

My child grinds their teeth. Do they need a plate?

Grinding is common in children and most grow out of it as adult teeth come through. We usually monitor rather than treat. Mention it at their check-up so we can keep an eye on the wear.

How long does a bite plate last?

Years, typically. It depends on how hard you grind, which is the point: better the plate wears than your teeth. Bring it along to your check-ups and we'll tell you when it's due for replacement.
